Presented here is a bone marrow smear obtained from a 73-year-old man with BMG. There is a diffuse infiltration of normal-appearing plasma cells. BMG is present in approximately 3% of the population over 70 years of age. In general, it is not a premyelomatous condition and may exist in a person for many years without ill effect. It is generally an incidental laboratory finding detected by routine SPE. Patients with this condition should be followed up periodically with SPE to insure detection of the rare transition to MM.
