This slide demonstrates the Wright's stained preparation of an undifferentiated AMoL, FAB type M5a. M5a is characterized by blasts that are so undifferentiated that they could be identified as myeloblasts or lymphoblasts. They were identified as monoblasts by cytochemical staining for esterase activity with alpha-naphthyl butyrate. The blasts also were positive for naphthol AS-D acetate esterase activity, and the enzyme was inhibited by NaF. A stain for MPX activity revealed less than 3% myelocytic elements. It would be extremely difficult to classify this leukemia without cytochemical staining.
